General Nursing Midwifery (GNM) Certificate
A. The Diploma in
General Nursing and Midwifery Certificate to the Old Syllabus students will
be issued on receipt of the proposals from the respective Principals of the
Nursing schools in the prescribed application along with the following
documents : The prescribed
application form duly filled in all the columns.
One photo of the Students taken in white uniform.
Xerox copy of the SSLC (10th Standard) Marks
Card, P.U.C (10+2) Marks card.
Xerox
copy of the Karnataka Nursing Council Registration Certificate.
Xerox copy of the
3 Years GNM Marks Cards.
The fee charged is Rs.300-00 per Student to be paid in any of the State Bank
of Mysore and Board copy of the Challan should be sent along with the
proposal.
B. The Diploma
Certificate to the New Syllabus GNM Students ie. from 05DN (2005) batch
onwards will be issued on receipt of the Internship Marks Card, Course
Completion particulars in the prescribed proforma (available in download
page) from the principal. There is no need to send the documents like marks card,
photo, SSLC and PUC marks etc.
The fee charged is Rs.500-00 per Student (Rs.100
per Internship Marks Card, Rs.100 Course Completion Certificate and Rs.300
for Diploma Certificate)
The Principal should collect fees from all the
passed students only and Payment made to the Board, in the form of D.D.
drawn in favour of
Medical Superintendent and Deputy Chairman, KSDNEB, Bangalore,
through the respective Principals of the Nursing Schools.
